Terms and Conditions for $100 Gift Card Survey Incentive:

1. Eligibility: This incentive is open to individuals who have received an invitation to participate in the "Your Moments, Our Mission" survey from The Tea House on Los Rios. Employees of The Tea House on Los Rios and their immediate family members are not eligible to participate.

2. Incentive Details: Participants who complete the survey will be entered into a draw for a chance to win a $100 gift card redeemable at The Tea House on Los Rios. The gift card is subject to the terms and conditions as stated herein.

3. Selection Process: The winner of the $100 gift card will be selected at random from the pool of eligible survey respondents. The draw will take place within 30 days after the survey closing date.

4. Notification: The winner will be notified via email and must respond within 14 days to claim their gift card. In the absence of a response within this period, an alternate winner may be selected.

5. Conditions of Use: The $100 gift card is non-transferable and cannot be exchanged for cash. The gift card must be used in accordance with the terms and conditions applicable at The Tea House on Los Rios, including any expiration date.

6. Privacy: Personal information collected from survey participants will be used solely for the administration of the incentive and will not be shared with third parties, except as necessary for the fulfillment of the incentive.

7. Liability: The Tea House on Los Rios is not responsible for any incorrect or inaccurate information provided by survey participants, nor for any technical or human errors that may occur in the processing of survey submissions and gift card distribution.

8. Changes and Termination: The Tea House on Los Rios reserves the right to modify, suspend, or terminate the gift card incentive at any time without prior notice, in compliance with applicable law.

9. Governing Law: This incentive and its terms are governed by the laws of the jurisdiction in which The Tea House on Los Rios operates.

10. Acceptance of Terms: Participation in the survey and acceptance of the gift card constitutes full acceptance of these terms and conditions.
